Medication Agreement
Patients of The Pain Rehabilitation Group that have been identified as candidates for opioid medication therapy are required to sign and adhere to our "Medication Agreement". This agreement is subject to revisions, and must be signed annually by all patients.
Patients are urged to familiarize themselves with the Medication Agreement and ask any questions they have concerning their pain management treatment. A copy of this document is given to the patient at the time of signature and scanned into patient chart.
