We look forward to becoming part of your health care team! 

If you have not received a copy of our new patient package in the mail, please download and print out the new patient paperwork PDF and complete all pages prior to your appointment.  Failure to complete paperwork will result in rescheduling of appointment.  

You will also receive an email from Therapractic that must be completed in addition to the new patient paperwork.  If you did not receive this email, or do not have access to a device to complete this, you will be able to complete at registration.

New patients should arrive 30 minutes prior to their schedule appointment time so that our staff can complete the registration process.   

Our staff will call to confirm your appointment one-two business days prior to your scheduled appointment. We must be able to verbally confirm this appointment with you. If we are not able to verbally confirm your appointment, your appointment will be cancelled. 

Please make sure that we have your correct day time phone number and you respond accordingly. If you are unable to keep this appointment please contact our office as soon as possible so that we can utilize this time for another patient.

Patients who fail to show for a scheduled appointment or give less than 24 hours notice of cancellation will not be rescheduled at this clinic.
